Depakote, an anticonvulsant typically prescribed in the United States for migraines, seizure disorders and manic episodes associated with bipolar disorder, received a black box warning from the Food and Drug Administration (FDA) in 2006. The black box label contains information and warnings for the following risks:

Neural tube defects: Use of Depakote can produce neural tube defects (such as spina bifida). Because of the risk, it is recommended that the benefits of Depakote use in women of childbearing age be weighed against the potential risks to unborn fetuses. The warning goes on to state that this extra caution is especially important in cases where Depakote is being used to treat migraines, as migraines do not typically pose a risk of permanent injury or death.

Hepatic failure: Use of Depakote can result in liver failure. Children under the age of two years are at increased risk of hepatic failure under certain circumstances. Parents should consult with their child’s doctor to discuss the risks associated with use of Depakote. Again, the benefits should be weighed against the risks. Studies have shown that the risk of liver failure drops considerably in progressively older groups.

Pancreatitis: Incidences of life-threatening pancreatitis have been reported for both adults and children after use of Valproate. Cases have been reported from as early as right after initial use to several years after initial use. Abdominal pain, nausea, vomiting and/or anorexia are all symptoms of pancreatitis and a doctor should be consulted immediately if these symptoms appear.
